Empowering lives through compassionate care and holistic support, Five Horizons Health Services Behavioral Health Department is dedicated to enhancing the mental and emotional well-being of individuals. Guided by a commitment to inclusivity, dignity, and community, we strive to provide comprehensive behavioral health services that address the unique challenges faced by our clients. Through evidence-based interventions and bias conscious approach, we aim to foster resilience and promote mental wellness. Our mission is to create a safe and nurturing environment where individuals can thrive emotionally, enabling them to lead fulfilling lives and contribute positively to their communities.

 

How Can We Help?

At Five Horizons Health Services, we recognize that life's challenges can often feel overwhelming, and seeking support is a brave and important step. Our compassionate therapists offer a safe, judgment-free space to help you navigate emotional and mental health concerns, including anxiety, depression, trauma, and the complexities of living with a chronic medical condition. 

What to Expect From Therapy?

1.) Initial contact: The first step in accessing care begins with a referral from our Clinical Team, Social Services Team, or Prevention + Outreach (P+O) Team. Once the referral has been received, please allow 1-3 business days for a member of the Behavioral Health Team to contact you by phone.

2.) Your First Visit: Your intake appointment, which typically lasts 60 minutes, will be completed by a member of my Behavioral Health Team. Please be prepared to complete intake forms, answer questions about your mental health history, and discuss your therapy goals. This appointment will help our BH team to understand your unique needs, allowing us to create a personalized treatment care plan just for you.

3.) Your First Psychotherapy Appointment with Your Therapist: Our team consists of experienced clinicians who will use evidence-based treatment approaches to help guide you through your therapy goals. After initial contact is made, you can expect to be scheduled for your first behavioral health psychotherapy appointment within 1-3 weeks. Session frequency typically occurs every 2, 3, or 4 weeks, depending on your individual needs.

 

Five Horizons Health Services does not currently offer 24/7 or after-hours support. 

Emotional Support Animals (ESA):

FHHS does not provide recommendations or letters of support for Emotional Support Animals (ESA).
